Confirm your Pathology Diagnosis

To request a second opinion (consultation) for your pathology case you could do one of the following:

- Obtain your slides from your pathology department, fill the downloadable 'Service Request From' found on this page, and mail all material including a check or money order to the address listed below. (preffered method for faster service) OR

- Fill the downloadable form, mail it along with a check or money order to the address listed below. We will then request your slides from your pathology department for review. (a $25.00 surcharge will apply to cover shipping expenses)

Fees for Cancer Second Opinion Consultation Service

• Full Pathology Report .........................................................................$150.00
  (slides provided for review- you would send the slides/reports to our lab)
  Includes a full pathology report examination of the slides, or analysis of the lab reports by an American Board of Pathology Certified Pathologist. The results will be given in a full official pathology report.
  
  
• Full Pathology Report .........................................................................$175.00
  (slides NOT provided for review- you want Confirmpath to request your slides/reports)
  We will request your slides for review from your pathology department. A $25.00 surcharge will apply to cover the test material request from the original pathology lab. This surcharge would also cover shipping expenses.

Payment forms accepted:

• Personal Checks. (Preferred method of payment. A $40.00 charge would apply to returned checks for insufficient fund)
• Money Orders.
• PLEASE DO NOT MAIL CASH.

ALL REQUEST FORMS SUBMITTED WITHOUT PAYMENTS WILL BE RETURNED TO SENDER WITHOUT EXAMINATION.
